Enrolling is easy - Subscribers and all enrolled family members must be covered under the same PEBB medical and/or dental plan. If both you and your spouse or qualified domestic partner are eligible employees or retirees, you will need to make enrollment decisions as PEBB does not allow dual coverage. Enrolled family members may be listed under one account, but not both. This means you could waive or defer the medical coverage on your account and be enrolled on your spouse's or qualified domestic partner's account, or enroll under separate accounts. You will need to decide who will cover any eligible dependent children.

- Find out if your provider participates with the medical plan you choose by checking the provider directory or contact the plan directly. If you are choosing a doctor or other provider for the first time, be sure to find out if he or she contracts with your plan choice and is accepting new patients.

- Choose your medical and dental plan. There are no restrictions or waiting periods for pre-existing conditions under any of the PEBB medical or dental plans.

Complete the enrollment form(s)
and any required dependent certification form(s).
Note: Enrollment deadlines are different for employees and retirees.
After you enroll, you'll receive an identification card from your plan. Show this card to your providers when you receive care. If you have any questions about your identification card, contact your plan directly. The Uniform Dental Plan does not issue identification cards.
Note for retirees: If you or any covered dependents are entitled to Medicare, you must enroll in Part A and Part B. Please include a copy of the Medicare card with your enrollment form(s).
Enrollment Deadlines
Employees
You must enroll yourself and your eligible dependents in a PEBB health plan within 31 days of the date you first become eligible for employer-sponsored benefits under PEBB rules. Complete the enrollment form(s) and return it to your payroll, personnel, or benefits office within 31 days. If you fail to submit an enrollment form within that period, you will automatically be enrolled in the Uniform Medical Plan Preferred Provider Organization (UMP PPO) and Uniform Dental Plan as a single subscriber. Claims for benefits under those plans will be denied until you submit a completed enrollment form. Your next opportunity to change plans or add dependents will be the next open enrollment period. For exceptions, refer to changing your plans.
You may enroll in additional amounts of optional life insurance, without health underwriting, within 60 days of becoming eligible for PEBB benefits and you may enroll in the optional long term disability coverage, without health underwriting, within 31 days of becoming eligible for PEBB benefits. You may apply, at any time, for optional life or long term disability insurance by providing evidence of insurability and receiving approval from the contracted vendor.
Long term care insurance is also available with limited health underwriting if applied for within 31 days of becoming eligible for PEBB benefits or at any time by providing evidence of insurability and receiving approval from the contracted vendor.
Retirees
You must return your completed enrollment form within 60 days after the date your active employer or continuous COBRA coverage ends to receive or defer coverage. Please note: If you or your dependents waived medical coverage during your active employment, you can enroll in or defer PEBB retiree coverage within 60 days after the date your active employer coverage ends. You may want to complete your enrollment form when you file your application for retirement benefits. If you choose to enroll in retiree coverage, your coverage will be retroactive to the date your active employee coverage ends.
If you don't enroll in or defer coverage within the 60-day period, you forfeit all further rights to enroll in the PEBB program.
Don't forget, you must enroll in Medicare Part A and Part B (if you're entitled) before you enroll in PEBB coverage.
